Our Duties
Elite Mind Wellness is required by law to maintain the privacy of your protected health information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. We reserve the right to change this Notice and to make the revised Notice effective for all PHI we maintain; an updated Notice will be posted on this website.

Your Rights Regarding Your Health Information

- ✓Amendment — request that we correct PHI you believe is inaccurate or incomplete.
- ✓Restriction — request a restriction on certain uses and disclosures of your PHI.
- ✓Confidential communications — request that we communicate with you by alternative means or at an alternative location.
- ✓Accounting of disclosures — request a list of certain disclosures we have made of your PHI.
- ✓Paper copy — obtain a paper copy of this Notice upon request, even if you agreed to receive it electronically.

Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at hello@elitemindwellness.com or with the U.S. Department of Health and Human Services, Office for Civil Rights, at www.hhs.gov/hipaa/filing-a-complaint. We will not retaliate against you for filing a complaint.
