Our Duties

Elite Mind Wellness is required by law to maintain the privacy of your protected health information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. We reserve the right to change this Notice and to make the revised Notice effective for all PHI we maintain; an updated Notice will be posted on this website.

How We May Use and Disclose Your Health Information

• ◇Treatment — we may use and share your PHI with providers involved in your care, including for coordination, referrals, and prescriptions.
• ◇Payment — we may use your PHI to bill and collect payment for the services we provide, including issuing superbills at your request.
• ◇Healthcare Operations — we may use your PHI for quality review, training, compliance, and other operations of our practice.
• ◇As Required by Law — we may disclose PHI when required by law, including for public health activities, court orders, and limited law-enforcement purposes.
• ◇With Your Authorization — any other use or disclosure, including most uses of psychotherapy notes and any marketing or sale of PHI, requires your written authorization, which you may revoke at any time.

Your Rights Regarding Your Health Information

• ✓Access — request to inspect and receive a copy of your PHI.
• ✓Amendment — request that we correct PHI you believe is inaccurate or incomplete.
• ✓Restriction — request a restriction on certain uses and disclosures of your PHI.
• ✓Confidential communications — request that we communicate with you by alternative means or at an alternative location.
• ✓Accounting of disclosures — request a list of certain disclosures we have made of your PHI.
• ✓Paper copy — obtain a paper copy of this Notice upon request, even if you agreed to receive it electronically.

How to Exercise Your Rights

To exercise any of these rights, contact our Privacy Officer at hello@elitemindwellness.com. We may ask you to submit your request in writing and to verify your identity.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at hello@elitemindwellness.com or with the U.S. Department of Health and Human Services, Office for Civil Rights, at www.hhs.gov/hipaa/filing-a-complaint. We will not retaliate against you for filing a complaint.
