Not a substitute for emergency care
Elite Mind Wellness provides non-emergency outpatient psychiatric services. If you are experiencing a psychiatric emergency — including thoughts of suicide or self-harm — call or text 988 (Suicide & Crisis Lifeline) or go to your nearest emergency room immediately.
Telehealth Consent
By using our services, you consent to receiving psychiatric care via a HIPAA-compliant telehealth video platform. You understand that:
- ◇Telehealth has inherent limitations compared to in-person care, including the inability to perform physical examinations.
- ◇Technical failures (poor internet connection, platform outages) may interrupt sessions. In these cases, we will reschedule at no additional charge.
- ◇You must be physically located in a state where our provider holds an active license at the time of each appointment.
- ◇A separate, detailed telehealth informed consent document will be provided before your first clinical appointment.
Payment Terms
Elite Mind Wellness is a cash-pay practice. We do not bill insurance directly. Payment is due at the time of service. We accept major credit cards, debit cards, HSA, and FSA funds.
Upon request, we will provide a superbill — a detailed receipt containing the procedure and diagnosis codes required for you to submit a claim directly to your insurance provider for potential out-of-network reimbursement. Reimbursement is not guaranteed and depends on your individual plan.
Our cancellation policy: appointments cancelled or rescheduled with less than 24 hours' notice may be subject to a cancellation fee. Details are provided at scheduling.
Geographic Limitations
We can only treat patients who are physically located in a state where one of our licensed psychiatric providers holds an active license at the time of service. This is a legal requirement of telehealth prescribing, not a policy choice.
Currently, Elite Mind Wellness is licensed and accepting patients in California and New Mexico. We are actively pursuing licensure in additional states. Using or attempting to use our services from a state where we are not licensed is prohibited and may result in cancellation of your account.
Eligibility
To use our services, you must:
- ◇Be 18 years of age or older, or a minor whose care is arranged and consented to by a parent or legal guardian.
- ◇Be physically located in a state where one of our providers is licensed at the time of each appointment (California and New Mexico).
- ◇Have a reliable internet connection and a device with a camera and microphone suitable for a secure video visit.

Governing Law
These terms are governed by the laws of the State of New Mexico, our primary state of licensure, without regard to its conflict-of-law provisions. Nothing in these terms limits any rights you may have under the consumer-protection or healthcare laws of your own state of residence.

We may update these terms from time to time. The “Last updated” date at the top of this page reflects when changes were last made. Continued use of our website or services after changes constitutes acceptance of the updated terms.
